Thank you for participating in our retrospective clinical trial. The purpose of the study is to assess your preference between Dysport vs. Botox for wrinkle reduction. Since you have received injections of both products from our office, you are being contacted to complete this study.

Please refer to the letter you received.. The letter may indicate that you had more than one area treated. If the letter indicates more than 1 treatment area, please complete an additional survey per treatment area.

It is our hope to publish this survey data in a medical journal to help document patient preferences between the 2 products, Dysport and Botox.
